What is PrEP'D?


Get PrEP'D is a national Australian initiative for gay, bisexual, and other men (cisgender or transgender), as well as non-binary individuals, who have sex with men. It aims to raise awareness about PrEP (Pre-Exposure Prophylaxis).


What is PrEP?


PrEP stands for Pre-Exposure Prophylaxis. It is a pill that HIV-negative individuals can take to prevent HIV. When taken before potential exposure to HIV, PrEP ensures there is enough medication in the body to block the virus if it enters. When followed as prescribed, PrEP has proven to be highly effective in preventing HIV, reducing transmission risk by nearly 100%.


Who is PrEP for?


PrEP is recommended for anyone at risk of HIV. Men who have sex with men (including trans and non-binary individuals) may be at risk if:

  • You’ve had or may have condomless sex (or if a condom breaks or slips off) with casual partners.
  • You’ve had or may have condomless sex (or if a condom breaks or slips off) with an HIV-positive partner who is either not on treatment or has a detectable viral load.
  • You use recreational drugs like crystal methamphetamine or GHB.
  • You’ve recently had an STI such as anal gonorrhoea, chlamydia, or syphilis.


How Can I Start Taking PrEP?


To start taking PrEP, you must visit a doctor to get a prescription. Any GP can prescribe PrEP. Before starting, you must be HIV-negative, so an HIV test is required. Your doctor will also perform STI tests and check your kidney health. Once on PrEP, you’ll need a sexual health check every three months when renewing your prescription, and your doctor may monitor your kidney function.


What About Other STIs?


PrEP protects against HIV but not other STIs or pregnancy. Regular sexual health checks are crucial for anyone sexually active. If you’re on PrEP, these STI tests become part of your routine visits to the doctor, ensuring any STIs are detected and treated promptly. Many STIs show no symptoms, which is why consistent testing is vital.


How Should I Take PrEP?


PrEP can be taken in different ways. The drug remains the same, but the method of intake varies depending on your needs.


Daily PrEP

  • For cisgender men who have sex with men: Take two pills at least 2–24 hours before sex to ensure high protection, then one pill daily.
  • For transgender or gender-diverse individuals: Daily PrEP is recommended to ensure full protection, especially if taking gender-affirming hormones or engaging in vaginal/front-hole sex. Take one pill daily for a full seven days to achieve full protection, then continue with one pill every day.

Daily PrEP can also be taken for specific periods, such as during holidays, summer, or events like Pride.


On-Demand (Event-Based) PrEP

If you're not having frequent sex and prefer not to take pills daily, on-demand PrEP might be suitable. However, you’ll need to plan for when you expect to have sex or be able to delay it by at least 2 hours. Adherence to the dosing schedule is crucial for effectiveness. Missed doses can reduce its ability to prevent HIV. On-demand PrEP is not recommended for transgender or gender-diverse individuals, as daily PrEP provides more reliable protection.


To take on-demand PrEP

  • Take two pills 2–24 hours before sex.
  • Take one pill 24 hours after the first dose.
  • Take one more pill 24 hours after (48 hours after the first dose).

A simple reminder: 2 + 1 + 1.

If you have sex again during this period, continue taking one pill daily until two days after the last sexual encounter.

Other HIV Prevention Options


In addition to PrEP, there are other options to prevent HIV:

  • Condoms: When used correctly, condoms effectively prevent HIV and pregnancy while reducing the risk of other STIs (though not eliminating it).
  • Undetectable Viral Load (UVL): People with HIV who are on treatment and have an undetectable viral load cannot transmit HIV to their partners. This is known as Treatment as Prevention (U=U or undetectable equals untransmittable).
  • PEP (Post-Exposure Prophylaxis): If you think you’ve been exposed to HIV, PEP is a treatment course that should begin as soon as possible after exposure. It is most effective when started within 24 hours but can be taken up to 72 hours after exposure.

Who should take PrEP?


A recommendation from the U.S. Preventive Services Task Force that urges doctors to prescribe a daily pill for all people with known risk factors for HIV could significantly raise awareness about an effective yet little-used drug. 

PrEP may be right for you if you test negative for HIV, and any of the following apply to you:
You have had anal or vaginal sex in the past 6 months and you

  • have a sexual partner with HIV (especially if the partner has an unknown or detectable viral load),
  • have not consistently used a condom
  • have been diagnosed with an STD in the past 6 months.

You inject drugs and you

  • have an injection partner with HIV, or
  • share needles, syringes, or other equipment to inject drugs (for example, cookers).

You have been prescribed PEP (post-exposure prophylaxis) and you

  • report continued risk behavior, or
  • have used multiple courses of PEP
